Don't remember the exact details of your setup. However, if you have the forums set to allow image uploade with posts/profiles then the rule is pretty simple:

One image per post.
One image per profile.

Ancient versions of the software had a bug not deleting older images if a new one was uploaded with a different extension. E.g. a user had a *.gif in his profile and changed to a new *.jpg, then the *.gif wasn't removed. So the remaining images on your server could be such stuff from years back running the old versions.

Other than that there is no issue.

The above one post / one image upload of course applies only to the basic Tetra-WebBBS version. The PRO extension is different.
